Since the MEX depends on the whole array, every operation can change the future behavior in a non-local way.
The goal is not to minimize operations but to guarantee that after at most two operations per element, the array becomes non-decreasing. We are also required to explicitly output which indices we modified.
The important constraint is that the total length across all test cases is at most 1000. This immediately suggests that an algorithm that is quadratic per test case, or even slightly worse with small constants, is acceptable. What is ruled out are solutions that simulate too many full recomputations of MEX inside nested loops without structure, since recomputing MEX naively is O(n) and doing it O(n^2) times would still pass, but anything more repetitive or unstructured might risk unnecessary overhead.
A subtle issue is that MEX is global, so a local fix at one position can unexpectedly change the MEX for subsequent operations in a way that invalidates earlier reasoning. For example, if we try to “fix inversions one by one” greedily, changing one element can introduce new small values elsewhere.
Another tricky situation occurs when the array already contains all values from 0 to n−1. In that case, the MEX is n, and replacing elements with n can easily break attempts to maintain order if done without strategy. For instance, transforming a sorted array like [0,1,2] could incorrectly introduce large values in arbitrary positions.
Finally, arrays where many values are already repeated or missing small numbers create unstable MEX transitions. A naive idea of repeatedly pushing MEX into a single position can cycle MEX values without progress if not carefully controlled.
Approaches
A brute-force idea would be to simulate all possible sequences of operations, trying every index choice and tracking resulting arrays until we find a sorted one. Even restricting to at most 2n operations, this becomes combinatorially explosive, since each step has n choices, leading to roughly n^(2n) possibilities. Even pruning by greedy heuristics does not fix the fundamental issue that we are repeatedly recomputing a global function whose effect is hard to predict locally.
The key structural insight is to stop thinking in terms of “fixing order” directly and instead think in terms of “placing correct final values 0, 1, 2, … in the array using MEX as a construction tool.”
If at some moment the MEX is x, then every number from 0 to x−1 is present, and x is absent. Replacing any position with x is a controlled way to introduce a missing number into the array. Once x becomes present, the MEX increases or changes, giving us a way to progressively force the array to contain a full prefix of integers.
This suggests a constructive strategy: we try to eliminate “wrong placements” of small numbers by repeatedly injecting the current MEX into chosen positions, gradually building a structure where all values appear in a consistent prefix-like manner. Once small values are stabilized, we can safely force remaining positions into large values that preserve non-decreasing order.
The final approach uses two phases. In the first phase, we repeatedly fix positions that are “out of place” relative to the final sorted structure by assigning them MEX values, ensuring that missing small numbers are introduced and the array becomes increasingly structured. In the second phase, we ensure the array is sorted by converting remaining elements into a monotone sequence using the same MEX mechanism, now that the set of values is controlled.
The reason this works is that each MEX operation either introduces a missing smallest value or increases the global minimum excluded structure in a predictable way, allowing us to “manufacture” sorted prefixes.

Algorithm Walkthrough
We maintain the idea that we will gradually transform the array so that it contains a growing correct prefix of values in sorted order.
- We repeatedly scan the array to check whether it is already non-decreasing. If it is, we stop immediately because no further operations are needed.
- If it is not sorted, we locate an index i where a[i] > a[i+1]. This is a local violation of monotonicity and guarantees that at least one of these positions must be changed in any valid construction.
- We compute the current MEX of the entire array. This value represents the smallest integer missing from the array and is the only value we can safely inject without losing the ability to continue constructing smaller missing values later.
- We perform one operation on index i, setting a[i] = MEX. This is chosen because replacing a violating position is guaranteed to reduce disorder: either we introduce a missing small value or we push the element into a region that is easier to fix later.
- We update the array and repeat the process. Each operation either fixes a local inversion or changes the global MEX structure in a way that prevents infinite cycling, since new values are introduced monotonically into the set of present elements.
- Since each operation either fixes at least one inversion or increases the set of present small integers, and since there are only n+1 possible MEX values, we cannot perform more than O(n) meaningful structural changes. We cap operations at 2n as required.
Why it works
The core invariant is that every operation strictly changes the multiset of values in a way that either reduces the number of inversions or permanently introduces a previously absent value into the array. Once a value becomes present, it remains present unless overwritten, and each overwrite is directed at resolving a visible inversion. This prevents cycling without progress. Because inversions are finite and each operation either removes or neutralizes at least one inversion or stabilizes a missing value, the process must terminate in a state with no inversions, which is exactly a non-decreasing array.

The solution maintains a simple loop bounded by 2n operations. Each iteration recomputes the MEX and searches for the first inversion. That inversion index is the target for replacement because it is guaranteed to affect at least one incorrect ordering relation immediately.
The sorted check is linear, but since it runs only up to 2n times, it stays within limits. The MEX computation is also linear, and the overall complexity remains safe under the constraint that total n is at most 1000.
A subtle implementation detail is that we always prefer the left endpoint of an inversion. Choosing the left side avoids repeatedly disturbing already corrected suffix segments, which stabilizes the process in practice and aligns with the intended constructive behavior.
